Adult Day Program

An adult day care program for financially eligible Ulster County seniors. Supervision, activities and a mid-day meals are provided. Call 845-334-5125 to make a referral.

There are two types of adult day care centers as defined by the New York State Department of Health: the medical model which provides medical monitoring, counseling, therapy and activities; and the social model, which provides socialization, supervision and activities. It offers respite to family members and caregivers, as well as providing a supervised setting when caregivers are at work or away during the day time. Within Ulster County, there is currently one medical model day care program (Ten Broeck Commons in Lake Katrine) and two social model day care programs (The Club at Golden Hill in Kingston and Valley Vista Adult Day Care Center in Highland).

Service(s) offered: Day care services providing medical monitoring, counseling, therapy and activities. Physical, occupational and speech therapy are provided, as directed by individual’s physician. As needed, blood work, X-rays, dental care and eye care can be provided. Continental breakfast, hot lunch and afternoon snack are included in the program.

Restrictions: Wheelchair-bound and/or incontinent individuals can register for the program. Patients must demonstrate medical necessity for admission.

Hours of operation: Program operates from 9:00 to 4:00, Monday through Friday. Transportation is not provided.

How Accessed: Physician’s written orders are required for admission to program.

Cost: Medicaid, Private pay.

Service(s) offered: The Adult Day Programs care for aging adults who are in need of assistance with activities of daily living or secure supervision. Delicious meals and a variety of supervised recreational programs. Also offering Activities, Meals, Games, Exercise/Dance, Pet/Music Therapy, Support Groups, Group Discussions, and Yoga/Qigong.

Hours of operation: 8:45 -4:00 Monday through Friday. Ask about transportation.

Cost: Managed Long Term Care and most major insurance accepted. Private pay options available.

Service(s) offered: Breakfast and lunch are provided daily to meet various diet plans. Services include physical exercise, social interaction, educational opportunities, memory enhancement activities, and independent living skills in an affordable home-like atmosphere. Activities include those which pique each individual's interest, and reduce stress. There is also on-site physical and occupational therapy providing education and training in daily living skills in the home.

Hours of operation: Program operates from 9:00 AM - 4:00 PM, Monday through Friday. Early drop off hours or late pick up hours must be pre-arranged.

Cost: Private pay. Participants may be eligilble for placement paid for by a Medicaid Managed Long Term Care Program (MLTCP)
