Case Management

Available to those with physical and financial need. These services include the Expanded In-Home Services for the Elderly Program, a uniform program of case management, non-medical in-home services, non-institutional respite and ancillary services, such as lifeline and safe return bracelets, for the elderly who need long term care but are not eligible for Medicaid. The case manager assists the client to apply for entitlements such as SSI, Medicaid, Food Stamps, HEAP (Home Energy Assistance Program) funds if they have no one to assist them. Call 845-334-5125.
